Kids 'r' Cookin' may wish to use photographs taken during our classes in our brochure, web site and/or advertising. Kids 'r' Cookin' has my express permission to use my child's photo as described above. Parent will be notified should your child's photo be chosen prior to the photo being used.

I, as parent or assigned guardian or caregiver of child named above, hereby acknowledge my understanding that he/she will be entering into and participating in those activities offered by Kids 'r' Cookin' in its children's cooking classes. These activities include, but are not limited to, engaging in cooking techniques, preparing various recipes and assisting in the preparation of food items. I further agree on behalf of my child to assume all risks associated with the above described activities and agree to hold harmless Kids 'r' Cookin', its owner, employees and representatives from any liability associated with such activities. My child has no condition, medical or otherwise, that would prohibit their full and complete participation in those activities offered by Kids 'r' Cookin'. In the case of an emergency situation wherein I cannot be reached, Kids 'r' Cookin' has my express permission to obtain all necessary medical attention for my child.
Refund policy: Full refund one (1) week prior to first class less $50.00 processing fee. Kids 'r' Cookin' reserves the right to cancel any class if a minimum of 6 students are not registered.
